Mare Owners please complete the following:
• Stallion & Mare Covering Return (White Copy)
• Mare Scanned in Foal Certificate
• Travel Grant Application Form (Copy on next page)
• Mare Swab Certificate (owners choice)

New Foal registrations
• Identification form – Completed by your vet
• Application & Owner Statement for registration form
• Suffolk Horse Blood Sample Record Sheet

AI Documentation
• Buying Suffolk Horse Society Semen Owner Declaration (must be completed before semen is sold.)
• Veterinary Surgeon Declaration. Insemination Report for Frozen Semen sold by the Suffolk Horse Society.
Embryo Transfer

Please note you are only entitled to Suffolk Horse Society grants if you a full member of the Society. If you receive a grant for coverings and a foal is born as a result, you must register your foal with the Suffolk Horse Society.
